SERVICE FEES, PAYMENT OPTIONS & POLICIES

I am excited that you are either considering or have already chosen to use naturopathic medicine for your health care needs. Here is some information regarding my policies, service fees and payment options here at Natural Family Medicine. Should you have any questions about the information provided here, please do not hesitate to contact me.

Service Limitations

I do not practice as a primary care physician and I do not provide emergency care services. I advise that you have an established relationship with a primary care physician for these services and for prescriptive needs. If you need a referral for this, please let me know. I would be happy to give you recommendations.

Fees

Service Fees

Service fees apply for consultations whether they are in person, by video, a phone call or by messaging through the patient portal. Charges are based on actual appointment time and is due at the time of service. There is no charge for phone calls or messages regarding questions pertaining to recommendations that I have previously made. However, if a message or phone call requires several minutes of my time, discusses new health concerns, or requires me to document new recommendations, service fees will apply.

Laboratory Testing Fees

Fees for laboratory tests are due when the order for the testing is placed. If I order laboratory tests, the appointment scheduled to discuss the laboratory results is a follow-up visit and service fees will apply. Please note that for every hour we spend together going over your results, I have already spent 1-2 hours putting together all aspects of your medical history in relation to these results to create a comprehensive plan for your health. In doing this, it minimizes the time that we take to review the results and discuss a treatment plan, reducing cost to you.

Supplement or Product Purchases

Fees for products or supplements are due at the time of purchase.

Appointment Cancellation Policy & Fee

If you need to cancel an appointment, kindly give 24 hours’ notice. A flat fee is charged for all missed appointments and cancelled appointments that occur with less than 24 hours of notice.

Payment Options

Types of Payment Accepted

I accept cash, check, debit and credit cards, including FSA & HSA cards and checks.

Insurance Coverage

Insurance companies do not cover naturopathic consultation services, laboratory orders or supplements in the state of Minnesota.

Health Savings Account

I will provide you with an itemized receipt and a medical necessity letter, if needed, that you may submit to your Health Savings Account (HSA) for reimbursement. I accept checks from HSAs and can charge HSA cards for services, labs and recommended supplements.

Consultations, laboratory testing and prescribed supplements are allowed on HSA programs as tax-free medical expenses

Product Returns

Adverse Reactions

If there is a product that I have recommended that does not set well with you or that causes an adverse reaction, please let me know by sending me a message directly through the patient portal or calling me at 612-250-2804. This type of return does require a brief follow up to document the reaction and discuss a suitable alternative product. If you purchased this item directly from my clinic within the last 30 days, we will apply an in-house credit to your account for the purchase price. if you purchased this item from my distributor, Fullscript, you will need to contact them for a refund.

Returning Unopened Bottles

Most unopened products purchased within the last 30 days can be returned. Heat sensitive products (digestive enzymes, oils, CBD products and probiotics) are not returnable. Products purchased longer than 30 days ago are not returnable. If you would like to return a product purchased directly from my clinic, please request approval by calling me at 612-250-2804 or sending me a message directly through the patient portal. If the return is approved, I will apply an in-house credit to your account for the purchase price. If you would like to return a product purchased from my distributor, Fullscript, you will need to contact them for a refund.

Products Purchased Outside of My Clinic

If you purchased a product that I recommended elsewhere, I can not honor a refund for any reason.
